Those taking dual credit in the Spring semester need to make sure to turn in the Dual Credit Request Form, W-9, and Bacterial Meningitis Forms.

She has to have the bacterial meningitis shot????
ReplyDeleteShe only has to have it now if she will be attending college courses on the college campus. That's why the form is the "Exemption Form" in the dual credit packet. She will have to have the shot before school starts in August.
